Dear Patients:

I am writing to inform you that I am retiring from active practice on December 31, 2024.  I have enjoyed taking care of your medical needs and want to make sure that you get the care that you will need going forward.

As of January 1, 2025, Dr Donald Peavy and Dr. Tom Curry will be available to continue your care at my current location on Bluebonnet.  Dr. Peavy has practiced ophthalmology in Baton Rouge for 31 years.  Dr. Curry previously practiced in New Iberia for over 30 years and is a new addition to Dr. Peavy’s practice.  They will be happy to continue your care, and I strongly encourage you to allow them to serve you. They can be reached at the same number of 225-766-1110 or the North Blvd office at 225-927-0180. Of course, you may choose a different ophthalmologist. 

You may contact your insurance company to obtain a list of ophthalmologists in the area who accept your insurance.  You can browse hospital staff listings on the internet.  If you have any specific needs or require guidance, you can call my office at 225-766-1110.  But be assured that your original medical records will be safely maintained for your convenience by Dr. Peavy and Dr. Curry for the prescribed length of time of 6 years since your last visit.

If you wish a copy of your medical records, we will offer the following services to you without charge (note, we will not mail records):

- For us to furnish your records to another physician or clinic, we require a written record release request. For your convenience, a record release form is available on this website below.  Provide the completed request form to my office granting me permission to share your records. You can send the completed form to us by mail (9237 Bluebonnet Blvd, Baton Rouge, 70810), fax (225-766-1112), email (eyedocagnes2002@gmail.com) or in person at the office.

 - For us to furnish you with a personal copy of your records, you may contact my office at 225-766-1110.  We will be happy to provide you with a copy for you to pick up at our office on Bluebonnet, or we can provide the records to you through your personal email or personal fax.

Again, my office at 9237 Bluebonnet Blvd will remain open fulltime with Dr Peavy, Dr Curry and staff to assist you going forward.
